iPad Parent Form for Grades 5-12

  • Parents/guardians of 5th-12th graders must complete the following paperwork and turn it in along with the annual $25 technology fee before an iPad can be issued. Fee discounts are available for those with more than one child in grades 5-12. The paperwork must be turned in by a parent/guardian to your child's school or submitted online below.

    All invoices by default will say $25 per child. The district is discounting fees per child based on the number of children you have in grades 5-12. Once you have completed the online Technology Registration form or turned in your paper form then we can go in and adjust your balance. If you would like to go ahead and make your iPad payment, all you need to do is open your invoice, choose Show Payment Plan, click Payment Options, and Pay Other Amount. The balance remaining will be adjusted as soon as the paperwork is processed.
